Start of the Geriatrics for Specialty Residents (GSR) program |
PURPOSE
The American Geriatrics Society (AGS), through a program funded by The John A. Hartford Foundation entitled Geriatrics-for-Specialists Initiative (GSI), seeks proposals for specialty-specific initiatives from academic training centers to develop, initiate and evaluate programs designed to increase education for residents in the geriatrics aspect of their disciplines. Dr. John R. Burton directs this program for the AGS.
BACKGROUND
The overall project addresses the urgent need to create a structure for developing leaders in geriatrics in academic surgery and related medical specialties. The Geriatrics for Specialty Residents (GSR) program allows interested specialists to work in collaboration with the geriatrics program in their institution, thereby enhancing their knowledge and skills in teaching geriatrics principles to specialist residents.
In June 2000, the Journal of the American Geriatrics Society published a special article entitled, "A Statement of Principles: Toward Improved Care of Older Patients in Surgical and Medical Specialties." The statement, developed by leaders from each of the ten specialties targeted under this initiative, has appeared in a number of participating specialty publications, sets forth the demographic and workforce issues that will impact on the provision of health care to older adults in the coming decades.
To meet this demand the "Statement" presented a set of objectives and recommendations for achieving them. The Geriatrics for Specialty Residents Program has been created as a response to the recommendation that we must "take steps to assure that specialty residency programs adopt specific learning objectives and curricula in geriatric care as part of a targeted effort to enhance residents' knowledge, skills and attitudes relevant to care of the older patient." in order to "Improve dissemination of new clinical research findings in geriatrics into the surgical and medical specialties."
PARTICIPATION
Proposals must be from institutions that have well developed academic programs in the applicant disciplines and in geriatrics. The proposal should be submitted by a member of the specialty department or division. Departments or divisions may submit only one application. Institutions may submit up to ten applications (one in each specialty). However, a maximum of two proposals from an institution will be funded.
FINANCIAL SUPPORT
Up to 15 two-year grants of $16,000 per year will be awarded to institutions that demonstrate in their proposals the most promise for success. No funds will be provided in support of indirect costs.

Abstract
In 1994 under the leadership of the late Dennis Jahnigen, the American Geriatrics Society, with support of The John A Hartford Foundation, began a project to improve the amount and quality of geriatric education that surgical and related medical specialty residents receive. The targeted disciplines initially were general surgery, emergency medicine, gynecology, orthopaedic surgery and urology and, later, anesthesiology, ophthalmology, otolaryngology, physical medicine and rehabilitation, and thoracic surgery. A key element of this project was to develop model programs within surgical and related specialty residency education. The Geriatrics Education for Specialty Residents (GSR) program has supported 29 residencies to pilot methods for integration of geriatrics within residency programs, has encouraged and inspired development of curricular content, and helped to develop faculty leaders to support these efforts long term and at a national level. This paper describes the GSR program, the status of curriculum development, steps for other programs to use in developing a geriatrics education program at their institution, and some of the common barriers likely to be encountered during implementation along with solutions to those barriers.
