Application Instructions

The application packet consists of an original, four collated copies and one disk copy of the following:

  1. Completed application form
  2. Proposal limited to 5 single spaced pages using no smaller than 11 point font including the budget. Your proposal should be organized in the following format:
    1. A brief description of your institution's residency training programs (number of programs, in what specialties, existing leadership or training programs for residents, etc…).
    2. How you anticipate the CRIT program will impact your institution.
    3. How the initiative will be sustained after June 30, 2010.
    4. A description of the program faculty.
    5. Other information you wish to share, if any.
    6. A description of the geriatrics program at your institution.
  3. Letters demonstrating explicit support of the proposed initiative from the Dean and residency training directors.
  4. Two-page curriculum vitae of the Principal and Co-Principal Investigators and any other key personnel.
  5. A stamped, self-addressed postcard to acknowledge receipt of your application.
