Checklist for Organizing an AGS State Affiliate

Obtaining Provisional Status

Decide on area for affiliate

Announce intent to organize by letter to all AGS members in the state or inter-state area

Publish announcement on intent to organize in AGS Newsletter

Hold first organizational meeting

____

Appoint an AGS contact at the organizational meeting

____

Fill out provisional status application form and return to AGS

____

Receive notification of provisional status from the national office

Obtaining Official Affiliate Status

Hold second organizational meeting

____

Elect officers and board members

____

Develop a committee to write by-laws and articles of incorporation

____

Approval of by-laws and articles of incorporation by affiliate membership

____

Approval of by-laws and articles of incorporation by AGS Board of Directors

____

Receive notification of affiliate status

____

Incorporate at the state level using a local attorney

____

Complete and file Form SS-4 (application for Employer ID Number)

Obtaining Tax Exempt Status

____

Obtain Federal Tax ID Number and 501 (c)(3) tax exempt status
