There are many reasons why I chose to complete a fellowship in geriatrics. On a personal level, I've always enjoyed being around older people like my grandparents and neighbors. So for me, specializing in geriatrics on a professional level was a natural fit. The quote, "...The harvest is plentiful, but the workers are few" rings true. As I watch my friends, family and the nation age, the need for caring, compassionate and competent geriatricians is critical. Job security is always a bonus, but the reality that my skills as a geriatrician are truly needed is very rewarding. In addition to my personal and professional interest in geriatrics, I also value continuing research to advance geriatric healthcare. So with my interest in geriatrics at heart, and the pressing need in sight, I chose to complete a 3-year combined geriatric/VA Quality Scholars fellowship at The University of Iowa and the Iowa City VA Health Care System.
The fellowship training program has provided a wonderful variety of clinical, educational and research oriented activities. In the clinics, I enjoy working within an interdisciplinary team, coordinating comprehensive care, and learning to address specific geriatric issues like: functional status, cognitive impairment, polypharmacy, advance directives, goals of care and end-of-life care. The challenge is very rewarding, as often small improvements in health status can have a tremendous impact on quality of life. In the academic setting, I have plenty of opportunities through lectures and discussions to help teach students, residents, fellows and faculty about special aspects of geriatric care. Since I have chosen a research-oriented fellowship, I will also complete a 2-year masters program in clinical investigation which includes coursework related to epidemiology, research design, data analysis and biostatistics.
Overall the geriatric fellowship provides the perfect combination of additional training, exposure to geriatric research and the opportunity to contribute to geriatric education. As a geriatrician with a background in internal medicine, I feel like I have plenty of career opportunities in the public, private and academic sectors of health care. My career plan currently is to complete the geriatric research fellowship and continue in academic medicine. Wherever I go, I hope to provide and improve geriatric health care.
