August 17th, 2010
An initiative created by a partnership of healthcare organizations, including the American College of Physicians (ACP), Georgia Academy of Family Physicians (GAFP), Georgia Healthcare Association (GHCA), and Georgia Medical Directors Association (GMDA) will help ensure that patients who are discharged from the hospital into long term care (LTC) facilities have the pain medications that are ordered for them.
On behalf of the partnership, AGS member Adrienne D. Mims, MD, MPH, Medical Director, Medicare Quality Improvement, Georgia Medical Care Foundation offers this helpful information that can be posted in newsletters and distributed to hospital discharge planners and physician documentation stations.
